We’re specialists in highly individualised dementia and cognitive disability care services.
We have care teams with specialist training in dementia care and cognitive disability supports.
We have a range of properties for Supported Independent Living including SDA properties available that are centrally located, with easy access to Perth and surrounding vibrant shopping centres.
Partnering in dementia
The Dementia Association of WA, focuses with us on powerful partnerships across Australia between all stakeholders. To provide leadership and diverse perspectives to design coordinated programs. The aim is to optimise innovation across sectors and disciplines, and enable research in complex support environments.
Continuity in dementia
We are aim to establish support infrastructure and processes that enable sustainable coordination and sharing of resources and data between dementia programs, in partnership with service providers and NDIS participants. 2Flourish supports innovation and research programs that incorporate continuity and cycles of learning and engagement.
Behaviours and training in dementia
We undertake a national approach to education including ability to recognise behavioural and psychological needs from individuals and families, dementia specific upskilling and professional development for the workforce providing dementia care in the community and in our accommodation settings.
Involving community in dementia
Strong consumer involvement including NDIS participants and stakeholders are important to us. This ensures programs and research opportunities can take place in an active partnership with those affected by dementia, so everyone has a say in setting research priorities, and generating and transferring knowledge.

Daily Living Activities | Home and Living Options
Daily Living is designed for everyday activities. Working towards your goals, we support your meal time preparation, showering and dressing and building capacity towards everyday home tasks. Services can range up to 24 hours in a day.

Getting out and about, meeting people, and trying new things! Welcome to Social and Community activities.

Support Coordination
NDIS Support Coordination, sometimes referred to as Case Management, involves assistance to support you and your family's ability to coordinate supports & participate in the community. This can include:
